The Electronic Funds Transfer (EFT) form is a simple, quick way to sign up for paperless bank payments. You can also to update or change this information after you've been set up.
To access the EFT form:
If you have not yet submitted EFT data, the full EFT form will display. Refer to Entering Electronic Funds Transfer Form Data for details on using the form.
Note: If you have previously submitted EFT data, clicking the Electronic Funds Transfer link from the menu will instead display the "Electronic Funds Transfer Updates" screen. Refer to Updating Electronic Funds Transfer Data for details on updating your information.
If you are a provider with more than one Tax ID Number (TIN), you can set up online EFT payment authorization separately for each TIN. You can then subsequently sign in using the Username and Password associated with each TIN.
